Minor Release Form
For good and valuable consideration, receipt and sufficiency acknowledged, I irrevocably grant Ingalls Pictures, LLC, a Michigan limited liability company, and its clients, affiliates, employees, contractors, representatives, licensees, successors, and assigns collectively, the “Released Parties,” the worldwide, perpetual, royalty-free, transferable right to photograph, film, record, edit, reproduce, publish, display, distribute, transmit, stream, exhibit, advertise, promote, sell, license, and otherwise use my child’s name, image, likeness, appearance, voice, statements, performance, signature, biographical information, and any materials my child provides, wears, performs with, or displays collectively the “Materials,” in connection with the project/session identified above.
The Materials may be used in any media now known or later developed, including broadcast, streaming, social media, websites, paid advertising, print, public relations, client presentations, trade shows, internal communications, training, fundraising, archival use, and any other commercial, promotional, editorial, or educational purpose. The Released Parties may edit, crop, retouch, caption, translate, combine, adapt, or otherwise modify the Materials, provided they do not intentionally misrepresent my child’s actual statements or actions.
I waive any right to inspect or approve the finished work or any use of the Materials, and any right to royalties, residuals, reuse fees, or other compensation, unless expressly provided in a separate written agreement signed by Ingalls. The Released Parties have no obligation to use the Materials or identify my child by name.
To the fullest extent permitted by law, I release and agree not to sue the Released Parties for any claim arising from the capture or authorized use of the Materials, including claims for invasion of privacy, right of publicity, defamation, false light, copyright, moral rights, performance rights, emotional distress, or similar claims. I also release claims arising from ordinary production activities, equipment, lighting, travel, location conditions, and communicable disease exposure, except to the extent caused by gross negligence or willful misconduct.
Ingalls may use ordinary production, editing, post-production, localization, accessibility, security, and workflow tools. Ingalls may not create a stand-alone digital replica or synthetic voice performance of my child for a new, unrelated production without my separate written consent.
This release applies only to Materials captured in connection with the project/session identified above, but the rights granted in those Materials are unlimited in time and territory. I represent that I have authority to sign as my child’s parent/legal guardian. I further represent that my child’s participation does not violate any contract, union obligation, employment policy, or other restriction known to me, and that I have any necessary authority or approval if my child appears on behalf of an organization or displays third-party names, logos, uniforms, instruments, artwork, trademarks, or materials.
This release binds me and my heirs, representatives, successors, and assigns, is governed by Michigan law, and may be signed electronically. If any provision is unenforceable, the remaining provisions remain effective.
By signing below, I acknowledge that I have read, understood, and agree to this release.
